Our client, a highly regarded not-for-profit organisation delivering frontline homelessness and community support services, is seeking an experienced Case Manager to join the House Case Management team within our Inner Melbourne Community Hub. This is an exciting opportunity to contribute to a leading organisation dedicated to supporting people facing homelessness, disadvantage, and complex needs.

As a Key Worker, you will provide holistic, strengths based case management to single adults residing in crisis accommodation. You will support clients experiencing homelessness and a range of intersecting needs, including mental health challenges, substance use, disability, justice involvement and family violence. Working within a multidisciplinary team, you will accompany clients through crisis, stabilisation and growth, helping them to access services, strengthen independence, and secure safe and affordable housing.


Duties and Responsibilities
  • Provide individualised, goal oriented case management to clients with diverse and complex needs.
  • Develop collaborative case plans that build on strengths and support housing stability.
  • Build a thorough understanding of housing options, referral pathways and community resources.
  • Liaise with internal and external partners including community housing providers, NDIS, mental health and AOD services.
  • Participate in the Homelessness Resource Centre roster and offer information, advocacy and practical support.
  • Maintain accurate records and ensure compliance with legislative, funding and organisational requirements.
  • Contribute to service development, continuous improvement, and organisational initiatives.
  • Promote a safe, respectful, inclusive and recovery oriented environment for all clients.
Skills and Abilities:
  • Bachelors Degree in Social Work or Equivalent 
  • Understanding of homelessness, trauma, mental health, substance use, disability, family violence and related service systems.
  • Experience providing case management and maintaining professional boundaries.
  • Excellent communication skills, including report writing.
  • A reflective, self-aware approach and willingness to learn.
  • Current Victorian Driver’s Licence
  • Working with Children Check
