Our client, a well-established not-for-profit organisation delivering critical homelessness and community support services across Victoria, is seeking a Community Connections Case Manager to join their Inner Melbourne Community Hub.
This role supports adults under 65 who are homeless or at risk of homelessness, including individuals sleeping rough and experiencing complex needs. Working within a multidisciplinary outreach team, you will deliver short- to medium-term intensive case management focused on housing stability and holistic support.
This role is 3 days per week and ongoing.

Duties and Responsibilities
  • Provide outreach-based, short to medium term intensive case management support
  • Work with clients experiencing homelessness or housing instability to address immediate and long-term housing needs
  • Develop and implement holistic, goal-oriented case plans to support sustainable outcomes
  • Coordinate access to housing options including crisis, transitional, public and private rental pathways
  • Liaise with external services including housing providers, mental health, AOD, NDIS, justice and family violence services
  • Conduct risk assessments and manage complex client interactions in community settings
  • Maintain accurate, timely case notes and data records in line with funding and compliance requirements
  • Participate in rostered support for clients accessing homelessness resource services
  • Contribute to service improvement, program reviews and best-practice delivery

Skills and Abilities
  • Relevant tertiary qualification with 1–2 years’ experience in community services (or equivalent experience)
  • Strong understanding of homelessness, housing systems and intersecting support services
  • Experience supporting clients with complex needs including mental health, AOD, disability and family violence
  • Ability to manage competing priorities in a high-volume, fast-paced environment
  • Strong risk assessment, judgement and professional boundary management skills
  • Excellent communication, documentation and stakeholder engagement abilities
  • Sound organisational and time management skills
  • Proficient in Microsoft Office and client management systems
  • Valid Victorian Driver’s Licence
