This role will see the successful candidate:
- work collaboratively with our Senior Medical Officer and General Practitioners and ensure the highest level of care is provided to our patients and reduce hospital admissions.
- triaging incoming requests from the RACFs allocated to you and conducting weekly onsite visits.
- coordinate care between GPs, aged care staff, residents, and families ensuring continuity of quality care.
- provide primary care services which may include:
- taking medical history
- undertaking clinical examination
- arranging any necessary investigations
- implementing a management plan
- providing appropriate preventive health care
- contribute to health assessments and medication management for new resident admissions.
- prepare annual health assessment reviews for existing residents.
- conduct regular reviews and follow ups with aged care residents through professional attendance.
- there may be opportunity to work from home once relationships with the nursing homes and patients are established.
Location - Dubbo - One day of work per week
- Condobolin - One day of work per week
- You will be reimbursed for your travel time and petrol.
- is registered with AHPRA and endorsed as a Nurse Practitioner.
- is presently permitted to work in Australia.
- is able to apply for a Medicare provider number.
- must have their own professional indemnity insurance (a copy of your policy must be provided to Ardent Medical for record purposes).
- a great attitude and able to work in collaboration with our Senior Medical officer.
- is committed to the company and a team player.
- has basic computer knowledge and good social / communication skills.
