Our client, a highly regarded not-for-profit organisation delivering frontline homelessness and community support services, is seeking an experienced  Team Leader – Community Connections  to lead an outreach case management team within Melbourne's North West. 

This is a great opportunity for a leader who considers themselves hands on as you will be overseeing the delivery of three programs and a small yet effective team.  In this role you will also hold a small case load, but primarily you will work to ensure the safety of your staff as they provide short to medium term outreach support to individuals at risk or experiencing homelessness in North West Melbourne. 

Duties and Responsibilities
  • Lead, mentor and support outreach case management team
  • Provide daily operational oversight, supervision, reflective practice and performance management
  • Ensure high-quality, holistic case management for clients with complex needs 
  • Assess and manage risk, critical incidents and complex client situations in an outreach settings
  • Provide secondary consultation and expert guidance on complex cases
  • Monitor service outcomes, data integrity, reporting and funding compliance
  • Contribute to continuous improvement, program reviews, audits and governance processes

Skills and Abilities
  • Relevant tertiary qualification (minimum Bachelor level) in Social Work, Community Services or a related field
  • Minimum 5 years’ experience in homelessness or community services, including leadership responsibilities
  • Demonstrated experience leading and supporting multiple programs and teams
  • Strong understanding of homelessness systems, housing pathways and intersecting services
  • Proven ability to manage risk, critical incidents and complex client presentations
  • Sound knowledge of compliance, governance and accreditation frameworks
  • High level communication, leadership and stakeholder engagement skills
  • Strong organisational, reporting and time management capabilities
  • Advanced IT literacy across case management and reporting systems
  • Demonstrated commitment to social justice, dignity, inclusion and trauma-informed practice
  • Valid Victorian Driver’s Licence
